Once logged in, click "Menu" then click "Manage Your Information" and lastly, "Patient Card"
After clicking "Begin Renewal," a pop-up appears for you to proceed with managing your card application. Click “Proceed” to begin your card renewal.
Scroll down while reviewing your personal information. If home address listed is correct, click "Apply." A pop-up window will appear, click "Apply" again. If you have recently moved and home address in incorrect, click "Go Back" and call Inspire Wellness at 877-793-5579.
Click "Begin Application."
Review your "Demographic Data" to confirm correct home address. Next, click "Continue." If your home address listed is incorrect, please DO NOT PROCEED and call 877-793-5579.
Click "Supply a Photo" to pull your Driver's License photo from DMV.
A pop-up appears after clicking “Supply a Photo." Next, click "Check State ID Photo." Next, click Proceed. Your Driver's License will now appear in the gray box. Lastly, click "Continue" to proceed to the next section.
Click "Supply Proof."
Click “Check State Residency Status” to automatically pull your residency status from DMV. Click "Proceed." Click "Continue" to the next section
Type in FIRST and LAST NAME and Click "Submit My Card Application."
After clicking “Submit My Card Application,” a pop-up appears informing you that a payment needs to be submitted. Click “Close.” Click "Continue" to the next and final section to make payment.
Click to Pay Online
Select "Credit Card." After typing in credit card information, click "Continue." On the next page, type in Email Address and Check Box. You will receive a Confirmation Number when completed.
Once payment is made, you will receive a Confirmation Number confirming completion of card renewal.
